YES, I am interested in participating in the City of Fairmont Adopt-A-Pot Program. I understand that if accepted, I will be responsible for the following four items: 1) DESIGN the planter (choose plants and flowers, to be reviewed by the City of Fairmont), 2) PURCHASE the approved flowers and plants for the planter, 3) PLANT the flowers and plants in the planter, and 4) PROVIDE MAINTENANCE (watering, weeding, etc.) for the planter through the end of the program, which will end in October. (NOTE - City of Fairmont Public Works will be responsible for delivering and picking up planters at the start and end of the season.)

Is the sidewalk wide enough to accommodate for the flower pot and not impede pedestrian traffic?
Yes
No
If the sidewalk is not wide enough, does it have adjacent hardscape (blacktop or concrete) that would allow for the planter to be placed adjacent to the sidewalk without impeding the use of the sidewalk or impeding vehicular or pedestrian traffic? And is the area flat?
Yes
No
Please list, in detail, the concept for the planter (types of plants, flowers, colors, number of each item, etc.)
I acknowledge and understand that the City of Fairmont is the sole owner of the planter and I will only be permitted to use it for a season. I understand that I will have to apply each year for future years I am interested in receiving a planter and will have to go through the process each year. I also understand that I am explicitly prohibited from painting or permanently modifying the planter and doing so will be see as vandalism or destruction of property and will be dealt with according to law. I also understand that The City, at their discretion, can remove the planter from in front of my business if it appears to have been vandalized, is not being properly cared for or maintained, without notice and without any financial reimbursement for incurred expenses for flowers, plants, water, etc. I understand that I am not allowed to plant anything that would be deemed illegal, broadly poisonous, or that could be considered a hazard to the public such as plants with large thorns, poison ivy, poison oak, marijuana, etc. By marking "YES" below I agree to these terms and conditions and all of the terms and conditions of the Adopt-A-Pot Program. I acknowledge that selecting "No" will mean immediate denial of my application.

Water levels must be monitored weekly- keep the planter full of water to avoid it getting too dry. This also will prevent it from being easy to move, remove, or be knocked over. The reservoir can hold 18 gallons of water at a time and during heat spells water can dissipate and be drawn up by the plants quickly. We do not recommend planting perennials in the planters unless you plan to remove them prior to pick up at the end of the season or do not care if they are disposed of at the end of the season. Should something happen and the planter need refilled with soil please contact the Public Works department to arrange delivery of additional soil. Planters must not be planted in a manner that makes it appear to be full of weeds. Plants must be very obviously a landscaping element that is beautiful and visually appealing. No garden flags or other garden type accessories will be permitted in the planters, with the exception of a trellis for vining type plants.
